NX Menu button and the top border bar are missing from the NX UI

After installing a new version of NX, the user reports that the Menu button and the top border bar under the ribbon are missing.

In a new default installation, NX runs with a simplified UI.  In the NX session, go to the Roles tab on the Resource Bar and choose one of the Advanced roles, such as "CAM Advanced" for a manufacturing user.  This will show the Menu button and its border bar, as well as all the extended ribbon bar icons the user is expecting to find.
