Note: This is usually due to a configuration problem where the FMS master config has not been updated with the proper volume/accesson id (requires an FMS restart).
It can also indicate a multisite configuration that is missing information about the given enterprise.

  - UGMASTER *is* attached to ItemRevision with Specification relation.







Solution

  - A misconfigured FCC process was running on the client machine.
  - After ensuring that FMS_HOME environment variable was set correctly,
    restarted the correct fcc process in a command prompt window

    cd /d %FMS_HOME%
    %FMS_HOME%\bin\fccstat -restart
