During a save/upload, modifed the Item's name ( object_name ) but the Item Revision's name did not update with the same change.
Solution
The Item Revision Name is not forced to match the Item Name.
If you want to edit the Item Revision Name you need to add it to the CPD (using attribute mapping to a custom SE property) and edit it directly. For example, with the following mapping:
