Most of the time the cause of the problem was the missing/incorrect rule for the particular extension. If the necessary extensions are already defined/added in the rules then try to upload the file manually via AWC. The problem can be caused by different things e.g. missing write access for the user or volume that is not correctly assigned, but uploading the file manually will usually reveal further error messages that can be used to better pinpoint the problem.
